If you activate a plan at the store, they charge you $35/line device connection fee (aka activation fee).

You will need a business name and an EIN. Personal SSN is not required. If you own a home business which does not have an Internet presence or if your business does not have a D&B profile, T-Mobile may require you to pay an advanced deposit. The deposit will be refunded back to you after 12 months.
You will have a direct account with T-Mobile.

We just help you and walk you through the process of establishing an account with T-Mobile. Then your account is handed over to you to manage. You are the owner of your own account. In return, we receive a compensation from T-Mobile. There is no cost to you. The process is much like if you purchase home/car insurance from an insurance agent. The agent helps you through the process. There is no extra cost to you as a customer, because the agent receives the compensation from the upper-level company.
The process of establishing a business account with T-Mobile is the following:
We will ask you for the basic business information, including business name, business EIN, your name, your phone number, your email, and the business address. Your personal SSN is not required, so there is no risk of leaking sensitive personal information.
The business information is then sent to T-Mobile's credit department for business underwriting. This process can take anywhere from a couple of hours to a week. The underwriting department verifies the business information against available Internet resources including Google, D&B, BBB, etc. If the business information can be verified easily, your account is ready in a couple of hours. If the business information is not readily verifiable, you may be required to pay a deposit. The deposit is paid directly to T-Mobile, and can be released after 12 months.
Once the underwriting is complete, you will receive an email from T-Mobile with the order information. You will verify the plan cost and submit the order.
Then the SIM cards will be shipped out within 1 business day with overnight shipping. If you are using eSIM, you can change physical SIM to eSIM in your T-Mobile online account portal. You can also port in your own number from another carrier after you receive your SIM cards. You don't have to port in beforehand when you open the account. You can port in at any time after the lines are already active.
