San Rafael Healthcare is a family owned and operated Pediatric Home Health agency in the greater San Antonio area. Our office is in the MEDICAL CENTER We are looking to hire a FULL TIME RN Case manager. Compensation package is competitive and includes flexibility and benefits. Case Managers manage a caseload of pediatric patients. The successful candidate will be accountable for the full patient care cycle. Case manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s health needs. We use KanTime for EMR documentation. Responsibilities -Manage a caseload of pediatric patients -Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered -Handle case assignments, create plan of cares, review and update paperwork as required/needed -Facilitate multiple care aspects (case coordination, information sharing, etc) -Develop effective working relations with office and field staff -Record cases information, complete accurately all necessary forms and produce statistical reports -Adhere to professional standards as outlined by protocols, rules and regulatory agencies -Rotating on-call schedule -Check off field nurses on their competency Skills -Proven working experience in case management, including as a nurse, medical, mental health care manager or a related job - Excellent knowledge of case management principles, healthcare management and reimbursement -Effective communication skills -Excellent organizational and time management skills -Problem solving skills and ability to multi-task -Compassionate with teamwork skills -Current active RN TX license -Pediatric Home Health experience is a plus!
